    • The Keto Diet is one of the most popular weight-loss diets the world over.

    • It helps in weight loss by achieving ketosis — a metabolic state where the liver burns body fat and provides fuel for the body, as there is limited access to glucose.

    What constitutes a keto diet?

    • A classic keto generally requires that 90 per cent of a person’s calories come from fat, 6 per cent from protein and 4 per cent from carbs.
    • But there are many versions doing the rounds since this one was designed for children suffering from epilepsy to gain control over their seizures.

    How does it impact the body?

    • If we starve the body of carbohydrate, after burning out the glucose, the liver starts breaking down fats for energy.
    • Ketosis is common in all kinds of fasting, but in a keto diet, when one is feeding it by giving a lot of fats from outside without carbs, it can become mildly toxic.
    • It may lead to many nutrient deficiencies such as carbohydrates, proteins, vitamins (especially vitamin A, D, E, & K) and minerals like calcium, phosphorus, sodium.
    • Extreme carbohydrate restriction can lead to hunger, fatigue, low mood, irritability, constipation, headaches, and brain fog, which may last days to weeks

    What impact does it have on our kidneys?

    • Even the moderate increase in protein needs to be carefully monitored, especially in those who are already suffering from chronic kidney disease, as it could lead to kidney failure.
    • One should get a thorough assessment and make sure they have normal kidney function before choosing this diet.
    • This diet could lead to increased stress on the kidneys and result in kidney stones, as they are made to work overtime.

    What is Brucellosis?

    • Brucellosis is a bacterial disease that mainly infects cattle, swine, goats, sheep and dogs.
    • Humans can get infected if they come in direct contact with infected animals or by eating or drinking contaminated animal products or by inhaling airborne agents.
    • According to the WHO, most cases of the disease are caused by ingesting unpasteurized milk or cheese from infected goats or sheep.
    • Symptoms of the disease include fever, sweats, malaise, anorexia, and headache and muscle pain.
    • While some signs and symptoms can last for long periods of time, others may never go away. Human to human transmission of the virus is rare.
    • These include recurrent fevers, arthritis, swelling of the testicles and scrotum area, swelling of the heart, neurologic symptoms, chronic fatigue, depression and swelling of the liver or spleen.
  • Panama Disease

    The scientists of Indian Council of Agriculture Research or ICAR have found a cure for one of the most dreaded diseases on Banana.

    Panama Disease

    • The fungal disease, called Fusarium Wilt, is popularly known as the ‘Panama Disease’ and afflicts banana plants.
    • For the first time, Indian scientists have brought out a biopesticide that can control the disease. This biopesticide has been made using another fungus.
    • For a long time, banana cultivators have been struggling with the Panama Disease.
    • This disease affects the Cavendish variety or the G9 Banana cultivar, which is the most widely grown banana in the world.

    Spread in India

    • In India, more than 60 per cent of bananas are of the G9 variety.
    • They go by names like ‘Grand Naine’, ‘Robusta’, ‘Bhusaval’, ‘Basrai’ and ‘Shrimanth’.
    • Farmers in at least four Indian states — Bihar, Gujarat, Madhya Pradesh and Uttar Pradesh — have been badly affected by this disease.
    • All these are areas where the Cavendish variety is grown.

    Why is the disease so deadly?

    • Panama Disease is caused by a fungus with a long and complicated name called Fusarium oxysporum f. Sp cubense.
    • One of its strains which is called ‘Tropical Race 4’ or ‘TR4’ is creating the most havoc, threatening almost 80 per cent of the global banana production.
    • The disease is so deadly that it is sometimes referred to as ‘banana cancer’.
    • The fungus resides below ground and infects the plant through its roots. The infection then stops water and essential nutrients from being transported to the rest of the plant.
    • The leaves begin to wilt, and the stem of the plant starts turning dark brownish before the plant dies. If one plant gets it, then it is most likely that an entire plantation can be wiped out.
  • What is a Serological Survey?

    A Serological Survey was recently conducted in New Delhi to determine the exposure of the novel coronavirus among the population.

    Serological Survey

    • A serological survey seeks to assess the prevalence of the disease in a population by detecting the presence of specific antibodies against the virus.
    • A serological test is performed to diagnose infections and autoimmune illnesses. It can also be conducted to check if a person has developed immunity to certain diseases.
    • The survey included the IgG Enzyme-Linked Immunosorbent Assay (ELISA) test which estimates the proportion of the population exposed to SARS-CoV-2 infection.
    • The IgG test is not useful for detecting acute infections, but it indicates episodes of infections that may have occurred in the past.
    • The test has been approved by ICMR for its high sensitivity and specificity.

    Highlights of the Survey

    • The study found the presence of antibodies in 22.86 percent of the people surveyed.
    • It indicated that a large number of infected persons remain asymptomatic.

    Why needed such survey?

    • Since it is not possible to test everyone in the population, serological studies are used as a tool to make an estimate of the extent of disease spread in the community.

    Conclusions from the survey

    • Results show that a significant proportion of the population is still vulnerable to contracting the novel coronavirus infection.
    • Containment measures need to continue with the same rigour.
    • Non-pharmacological interventions such as physical distancing, use of face mask/cover, hand hygiene, cough etiquette and avoidance of crowded places etc. must be followed strictly.
  • What is Itolizumab, newly cleared for Covid?

    The Drug Controller General of India recently approved Itolizumab as a novel biologic therapy for restricted emergency use.

    Various drugs in news these days are:

    1) Remdesivir: Ebola

    2) Lopinavir: HIV

    3) Hydroxychloroquine: Malaria

    What is Itolizumab?

    • Itolizumab is an existing drug used for psoriasis, a chronic skin disease involving unregulated growth of some skin cells that develop into red patches mostly on knees and elbows, but also on some other parts of the body.
    • The drug, developed by Bengaluru-based Biocon, was approved in 2013.
    • It is a monoclonal antibody which is used to treat acute psoriasis.

    Why it is being used for COVD-19?

    • The coronavirus has been observed to induce an overreaction of the immune system, generating a large number of cytokines that can cause severe damage to the lungs and other organs.
    • Itolizumab is safe and efficient in preventing cardio-renal complications in Covid-19 patients who also have acute respiratory distress.

    What are monoclonal antibodies?

    • Monoclonal antibodies are proteins cloned in the lab to mimic antibodies produced by the immune system to counter infection.
    • They have their genesis in serum, the colourless constituent of blood that contains antibodies.
    • These proteins bind to an antigen, the fragment of an infectious virus in the case of SARS-CoV-2, and either destroy it or block its action.

    • He viewed addiction as a medical condition rather than a moral failure.
    • He spent years treating people with drug addiction and realized that the treatment needed a new approach backed by scientific research.
    • His new methods of treatment gained an appreciation and he was appointed as the deputy director for demand reduction at the Office of National Drug Control Policy by the then U.S. President George H. W. Bush.
    • He headed many projects on developing new methods to treat individuals with alcohol, cocaine, heroin and alcohol addictions.
  • What is Foldscope?

    Indian researchers have explored and validated the clinical utility of Foldscope in the diagnosis of diseases using various patient samples.

    Though trivial, Foldscope is a significant invention with most crucial applications. It somehow offers an alternative to costly microscopes for some basic diagnosis.

    What is Foldscope?

    • Foldscope is an affordable origami-based microscopy device composed of a series of paper clippings.
    • Upon assembly, the device can hold a specimen slide for observation, and this specimen can be viewed via a mobile phone camera attached to it.

    How does it work?

    • Foldscope can be assembled using paper clips and mounted on a cell phone using coupler and glue drops.
    • To do the assessment, a patient sample like urine is smeared on a transparent glass slide and visualized under a Foldscope mounted on a cell phone.
    • Sample images can be enlarged using the zoom function of the mobile, which can be stored on the mobile memory card for later reference/patient records.
    • Foldscope visualizes calcium oxalate crystals, which are a major cause of kidney stones.

    Utility of Foldscope

    • Foldscope is particularly convenient to diagnose urinary tract infection (UTI) and monitor kidney stone.
    • The study evaluated the use of Foldscope in the clinical diagnosis of oral and urinary tract infections.
    • Using this tool, one can easily monitor own-kidney stone status at home with a simple glass-slide, a Foldscope and a phone in hand.
    • Such monitoring could perhaps avoid kidney stone reaching a painful state or surgery in recurring cases.
  • Crystalline Rubrene for Optoelectronic Devices

    Indian scientists have found a new process for synthesizing crystalline rubrene for the development of optoelectronic devices and also for preparation of Electronic Skin (E-Skin).

    Note the difference between the Pyro-electric/ Piezo-electric/ Pyro-photonic effects. UPSC may shuffle the meaning of such terms in statement based prelims question.

    What is crystalline rubrene?

    • Crystalline rubrene is a polycyclic aromatic hydrocarbon-based thin film.
    • It is a water-free, solvent-free, environmentally friendly one-step plasma process.
    • It demonstrated optoelectronic properties (which detect and control light).
    • A highly uniform pin-hole-free thin film can be deposited by this process, which is useful for the fabrication of high-end devices.

    Working principle

    • Devices made of pyro-electric materials (that generate electric charge when they are heated or cooled) and piezo-electric materials (that generate electric charge under the effect of mechanical pressure), can help detect change in temperature and pressure.
    • Pyro-electric materials also show pyro-phototronic effect where pyro-electricity is associated with the change in temperature of a material when it absorbs photons.
    • Pyro-electric infrared detectors are well known for application in infrared sensing for space research, defense, remote sensing, and household appliances.

    Principle application: Human Skin

    • These kinds of materials are available in biological systems such as – human skin, plant cellulose leading to their significance in the understanding of basic science of biological systems and also in their huge application prospect.
    • The rubrene crystal has a thin amorphous oxide layer formed over the crystalline film.
    • This induces surface layer polarization effect leading to pyro-phototronic effect.

    Significance of the research

    • Since last few years, scientists from around the world are working on the synthesis of organic materials for electronic applications.
    • The conventional process for synthesis of organic electronic materials based on chemical processes provides very good quality materials, but the stability of the materials is not very good, and it requires use of solvents.
    • Moreover, multiple steps are required for material synthesis and film deposition.

    Applications

    • This novel process developed by the Indian team is useful for developing advanced optoelectronic devices and preparation of Electronic Skin (E-Skin).
    • It may prove to be useful tool for laboratory simulation of different biological systems for probing the organization and dynamics of those systems.
  • Debate over a homoeo drug – Arsenicum album 30

    A homoeopathic drug, Arsenicum album 30, has become a subject of debate after several states and AYUSH Ministry recommended it for prophylactic (preventive) use against Covid-19.

    Arsenicum album 30

    • Arsenicum album is made by heating arsenic with distilled water, a process repeated several times over three days. The drug has less than 1% arsenic.
    • A small bottle with one course costs Rs 20-30.
    • Arsenicum album is considered to correct inflammation in the body. It takes care of diarrhoea, cough and cold.
    • It is used commonly by homoeopaths to treat anxiety, restlessness, cold, ulcerations, burning pains. It is taken in powder form or as a tablet.
    • The health hazards of arsenic contamination in water are well known: long-term exposure to the metal can cause skin cancer, pulmonary and cardiovascular diseases.
    • It has been recommended against COVID by the state governments in Rajasthan, Karnataka, Tamil Nadu, Andhra Pradesh and Kerala.

    The Covid-19 context

    • Arsenicum album 30 could be taken as prophylactic medicine against Coronavirus infections.
    • It is only “possible prevention” against flu.
    • The AYUSH Ministry recommended taking the medicine for three days on an empty stomach and repeating the dose after a month if an outbreak continues locally.

    Issues with such medicines

    • The WHO neither has any guidelines nor any effective evidence on using Arsenicum album as a Covid-19 treatment.
    • The debate stems from the fact that there is no scientific evidence that the drug works against Covid-19, a fact stressed not only by medical scientists but also by some homoeopathic practitioners themselves.
    • There have been reports about people flocking to homoeopathic clinics to buy Arsenicum album, sometimes at triple the cost.
    • Even local chemists have started stocking this medicine.
    • Self-medication can prove harmful as prevention or cure for COVID-19.
  • What is the FAITH’ Trial?

    With the number of COVID-19 patients rising in India, a pharma company has announced a new randomized study to test the combined efficacy of two antiviral drugs under the ‘FAITH Trials’.

    Misleading names: One may get confused over the names given to these clinical trials. The name ‘FAITH’ and ‘Solidarity’ appear more like a judicial trial or some Human Rights violation related trials. UPSC can knock such areas in prelims.

    FAITH Trials

    • The two drugs: Favipiravir and Umifenovir will be tried as a potential COVID-19 treatment strategy.
    • This new combination clinical trial will be called FAITH – (FA vipiravir plus Um I fenovir (efficacy and safety) Trial in Indian Hospital setting).
    • The two antiviral drugs have different mechanisms of action, and their combination may demonstrate improved treatment efficacy by effectively tackling high viral loads in patients during the early stages of the disease.
    • This trial offers a comprehensive antiviral cover on pre-entry and post-entry life-cycle of the SARS-CoV-2 virus.

    Dosages under the trial

    • Patients taking the drug will receive Faviprivir 1800 mg bid and Umifenovir 800 mg bid on Day 1.
    • Thereafter, they will receive Faviprivir 800mg bid and Unifenovir 800mg bid for the remaining course of treatment.
    • Duration of treatment will be 14 days and patients will be discharged after clinical cure and two consecutive negative tests.
    • While one group will be receiving Favipiravir and Umifenovir (with standard supportive care), the other group will receive Favipiravir along with standard supportive care.

    Other trials in news: The Solidarity Trial

    • “Solidarity” is an international initiative for clinical trials launched by the WHO, along with partners, to help find an effective treatment for Covid-19.
    • It was originally supposed to look at four drugs or drug combinations: Remdesivir, HCQ, Ritonavir/Lopinavir and Lopinavir/Ritonavir/Interferon beta 1a.
    • Now with HCQ trial enrolment stalled for at least the next few weeks, the Solidarity trial will proceed with the other three arms.
